New Patients

LATE APPOINTMENT POLICY:  If you show late to an appointment you will still receive a massage if time permits, or at a reduced time at the full charge.  More than 15 minutes late is considered a Broken Appointment.  If you are running late a phone call is appreciated. If the therapist is late then you will receive a reduction in fee for the session. BROKEN APPOINTMENT POLICY: At least 24 hour notice is required to change or cancel an appointment. If you fail to do this you are obligated to pay for the session in full.

New Patients: For your convenience, these forms are provided in PDF format. 

So that you may fill them out in advance and bring them with you to your first appointment with the practitioner.
